Class Policies
Please click on any of the links bellow to access electronic copies available
for Spectrum System users. You may
complete any of these forms online and then save them locally to your hard
disk.
To submit a form, you may
either include it as an attached file in your email to Shyamalie
Wijeratne (swijeratne@cis.gsu.edu), or you may print
out the form and submit it on paper. However, all forms must have a
signature. If you are submitting by email, you'll still need to drop
by Ms. Wijeratne's office to sign the form. Typing your name in for a
signature is not acceptable. Please be sure to save a copy of your
completed form on your hard drive.
